REGISTRATION FORM for KIDS CLASSES at Maureen's

CLASS FEES for Kids' Classes: Price listed is per class.  Adults and older children are welcome to attend these classes as well. Adults attending with a child will be encouraged to take the class and do their own project.

Class fees - $16 per class, per person (includes $10.66 for class, $5.00 materials fee, $.34 tax)

For early registrations (more than 6 weeks prior to class) a deposit of $5.00 per person is required at time of registration. 

If registering less than 6 weeks prior to class, full payment is required at time of registration.
